Port Washington Post Acute is actively hiring LPNs and RNs for day and night shift. 12 Hour shifts from 6 - 6.
We are a 98 bed skilled nursing facility under new ownership and new management. Major improvements are underway, including facility upgrades, stronger clinical systems, and a renewed focus on staff support.

LPN / RN responsibilities:
Administer medications and treatments safely
Complete assessments and document resident condition
Monitor changes in condition and notify providers as needed
Supervise CNAs and support direct resident care
Coordinate with therapy, providers, and interdisciplinary team
Complete admissions, discharges, and charting
Respond to changes in condition and emergencies
Support care planning and resident outcomes
Maintain infection control and safety standards
Qualifications:
Active LPN or RN license in Washington
Dependable attendance
Long term care or skilled nursing experience preferred, but not required
Strong clinical judgment
Strong teamwork and communication
