Community Healthcare Partners (CHP) is looking for a compassionate, organized, and patient-focused LPN or RN to join our growing healthcare team as a Patient Care Coordinator. In this role, you'll work closely with providers, clinical staff, and office personnel to ensure patients receive exceptional coordinated care. You'll serve as a primary point of contact for your assigned patients, helping them successfully manage their chronic conditions while ensuring timely communication between all members of the healthcare team.
What You'll Do
Coordinate Chronic Care Management (CCM) services for an assigned patient panel.
Review patient records and establish patients into CHP's CCM program.
Monitor patients' health status and identify changes that require provider intervention.
Educate patients on managing chronic illnesses, medications, treatment plans, and available healthcare resources.
Communicate with providers regarding patient concerns, lab results, medication changes, and new orders.
Coordinate care with specialists, home health agencies, pharmacies, durable medical equipment suppliers, and other healthcare partners.
Ensure individualized care plans are implemented and followed.
Collaborate with scheduling, medical records, billing, coding, and other office staff to provide seamless patient care.
Maintain accurate, timely, and compliant documentation of all patient interactions.
Respond to patient questions and acute healthcare needs with professionalism and compassion.
Perform other duties as assigned to support quality patient care.
What We're Looking For
Current Nebraska LPN or RN license in good standing.
Excellent communication and customer service skills.
Strong organizational and multitasking abilities.
Ability to work independently while collaborating with a multidisciplinary healthcare team.
Experience in primary care, case management, care coordination, home health, or chronic disease management is preferred but not required.
Passion for improving patient outcomes and providing exceptional patient-centered care.
What We Offer
Competitive Pay
LPN: Starting at $28.00 per hour plus CCM Bonus Program
RN: Higher starting wage based on experience, plus CCM Bonus Program
Pay with CCM Bonus Program averages $33-$38/ hour
