About This Position

<Job Summary>

Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, high-quality nursing services across diverse settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your skills will support patient assessments, medication administration, wound care, and vital sign monitoring, ensuring safe and effective treatment plans. This position offers an exciting opportunity to work with a multidisciplinary team dedicated to delivering exceptional care while advancing your professional growth in a fast-paced environment.

<Responsibilities>

Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health records.

Assist with Medications, diagnostic evaluations like specimen collection, laboratory processing, and medical imaging coordination.

Support patients with activities of daily living, including tube feeding, catheter care, wound dressing changes, and mobility assistance with Hoyer lifts or other equipment.

Document all patient interactions meticulously in medical records while adhering to HIPAA regulations to ensure confidentiality.

Collaborate with physicians and interdisciplinary teams in discharge planning, case management, and post-acute care coordination for diverse populations including pediatrics, geriatrics, and individuals with developmental disabilities or dementia.

<Skills>

Proven experience working with electronic health record.

Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and treatment.

Skilled in performing advanced procedures including and catheterization.

Experience providing specialized care such as Alzheimer's/dementia/memory care or hospice services; familiarity with infection control practices is required.

Ability to work effectively in fast-paced environments like acute care hospitals or outpatient clinics; previous home & community care experience is advantageous.

Excellent communication skills for triage assessments and patient education; competency in medical documentation and report writing is essential. Pay: $30.00 - $34.00 per hour
