Position Title: Primary Care Nurse (LPN)

Position Type: Full-Time, Non-Exempt

Shift: 12-hour shifts, Rotating Weekends

Day Shift, 6am-6:30pm

NOC Shift, 6pm-6:30am (+$4.00 NOC differential)

Position Summary:

Provides direct nursing care to residents, delegates and supervises the care provided by nursing assistants.

Qualifications:

Washington State LPN License (Required)

CPR certification (Required)

Continuing education for gerontological and/or long-term care nursing preferred.

Essential Duties and Responsibilities:

Meets with nursing assistant to assess, plan, and evaluate resident’s care and medications.

Completes daily skilled notes.

Documents nursing activities on appropriate forms.

Receives and records physicians’ orders.

Communicates staffing problems and needs to Charge Nurse.

Reports problems and complaints to Charge Nurse.

Provides feedback to Charge Nurse regarding performance of nursing assistants.

Receives/gives end of shift report to licensed staff and nursing assistants.

Assists in admission, transfer, and discharge of residents.

Recommend supply and equipment needs.

Participates in development, implementation, and evaluation of the quality assurance program.

Fill out incident report forms.

Greets and admits new residents.

Make rounds with physicians and other team members.

Assess residents.

Participates in development, implementation, and evaluation of residents’ care plans.

Coordinates residents’ care activities.

Identifies and reports changes in residents’ status.

Recommends and implements changes in the care plan.

Responds to emergency situations.

Gives treatments and other direct care.

Obtains specimens as ordered.

Prepares and administers medications as prescribed.

Identifies and promptly communicates adverse drug reactions.

Maintains narcotic records accurately.

Orders and arranges for ordering of pharmaceuticals.

Notifies Charge Nurse or supervisor of discrepancies in drug inventories.

Upon acceptance of an offer, candidates must be able to pass a drug screen, reference checks, and background check prior to starting work with eliseo. Upon first day of hire, all staff are required to provide documentation of influenza vaccination (Seasonally for Influenza). Additionally, all staff will undergo tuberculosis testing upon their first day and annually.

Pay: $36.29 - $44.11 per hour
