Position: Licensed Practical Nurse (LPN)
Speciality: Long-Term Care (LTC) / Nursing Home
Shift: Nights | 6:00 PM – 6:00 AM
Schedule: 12-hour shifts
Position Overview
Wakefield Health Care Center is seeking a compassionate and dependable LPN for a night-shift LTC assignment. The ideal candidate will be comfortable caring for long-term care residents, including individuals with dementia, while working collaboratively with CNAs and other members of the healthcare team.
Responsibilities
Provide quality nursing care to LTC residents during overnight shifts.
Administer medications and treatments as ordered.
Monitor residents and promptly identify changes in condition.
Complete accurate electronic documentation using PointClickCare.
Collaborate with CNAs and other healthcare professionals to ensure safe, compassionate patient care.
Support residents with a variety of long-term care needs, including dementia.
Follow facility policies, procedures, and safety standards.
Experience:
LPN-LTC: 1 year (Required)
License/Certification:
LPN any License or Compact state License (Required)
BLS from AHA (Required)
Work Location: In person
