Responsibilities
Provide comprehensive nursing assessments and develop individualized care plans for patients receiving home health or hospice services
Administer medications, treatments, and interventions following physician orders while monitoring patient responses
Educate patients and families on disease management, medication usage, symptom control, and post-acute care procedures
Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and utilization review
Conduct regular visits to evaluate patient progress, vital signs, and physiological status; adjust care plans accordingly
Maintain accurate medical records in compliance with HIPAA regulations and document all patient interactions thoroughly
Collaborate with physicians, social workers, therapists, and other healthcare providers to optimize patient outcomes in outpatient or inpatient settings
Requirements
Valid RN (Registered Nurse) license in good standing with relevant state licensing boards
Strong background in hospital medicine, medical-surgical units, or post-acute care settings including outpatient clinics or home health agencies
Knowledge of physiology, medical terminology, infectious disease protocols, and discharge planning processes
Experience with patient assessment techniques, vital signs monitoring, diagnostic evaluation, and case management strategies
Familiarity with HIPAA compliance standards and medical records documentation practices
Ability to work independently while managing multiple patient cases efficiently in a fast-paced environment and communicating with the each patients' healthcare team
