About This Position
Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. In this vital role, you will provide high-quality nursing care to residents in a skilled nursing or assisted living environment, ensuring their comfort, safety, and well-being. You will work closely with interdisciplinary teams to deliver personalized care plans, administer medications, and monitor residents' health status. Your expertise will help create a supportive atmosphere that promotes dignity and enhances quality of life for our residents.
Responsibilities
Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
Administer medications and treatments accurately, including injections, IV infusions, catheter care, and tube feeding, following established protocols and safety standards.
Monitor residents’ conditions vigilantly, recognizing signs of acute pain, infection, or deterioration; respond promptly with appropriate triage procedures.
Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans tailored to each resident’s needs.
Maintain detailed medical documentation in compliance with HIPAA regulations and long-term care regulations; ensure accuracy in ICD-10 coding and medical records management.
Provide education and support to residents and their families regarding health conditions, medication management, dementia care, Alzheimer’s care, and other specialized needs.
Ensure infection control policies are strictly followed; utilize aseptic techniques during procedures such as venipuncture or specimen collection.
Qualifications
Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in a long-term care setting.
Proven experience working with geriatric patients in skilled nursing facilities or assisted living environments.
Strong knowledge of long-term care regulations, dementia care practices, hospice care principles, and memory care strategies.
Familiarity with EMR/EHR systems such as Cerner or Athenahealth; experience with medical documentation and ICD coding preferred.
Competence in performing clinical procedures including catheterization, sterile processing, first aid, and basic life support (BLS).
Excellent communication skills for caregiver interaction, patient assessment, family education, and interdisciplinary collaboration.
Ability to handle emergency situations effectively; experience in acute care environments such as hospital medicine or Level I trauma centers is advantageous.
Knowledge of physiology, anatomy, pharmacology related to pain management and infectious disease care; experience working with individuals with disabilities is a plus.
