About This Position
Job Summary
We are seeking a compassionate and organized Registered Nurse (RN) Case Manager to join our home health team. The RN Case Manager is responsible for coordinating patient care, completing assessments, developing care plans, and ensuring patients receive high-quality care in their homes. Business hours: Monday–Friday, 8:30 AM to 5:00 PM.
Responsibilities
Perform patient assessments and develop individualized care plans
Coordinate care with physicians, therapists, and other healthcare providers
Provide skilled nursing visits and monitor patient progress
Educate patients and families on disease management and care plans
Ensure accurate and timely documentation in the medical record
Communicate patient status and updates to the care team
Qualifications
Active California RN license
Home health experience preferred
Strong clinical assessment and communication skills
Ability to work independently and manage a caseload
Valid driver’s license and reliable transportation
Pay: $45.00 - $55.00 per hour
