Job Summary
Under the supervision of the PACE Home Care Liaison, the Home Care Aide will provide home care support and help ensure the Plan of Care home care task developed by the PACE Interdisciplinary Team is being followed.
Education - High School Diploma or equivalent.
Experience - One (1) year healthcare experience with elderly preferred. One (1) year working with a frail or elderly polulation.
Other Requirement(s) - Ability to travel in the Seattle area for patient visits: active WA State Driver’s License and proof of insurance is required. Active Certified Nursing Assistant (CNA) license or qualified Nursing Assistant Registration or qualified Home Care Aid Certification required. Home Care Aid Certification within 200 days of date of hire.
