About This Position

Plan and provide professional nursing assessments for patients at UroHealth Partners clinics in accordance with Physician or Provider orders and established policies/procedures. The Triage Nurse (LPN/RN) is responsible for coordinating the delivery of patient care originating from the clinic setting and managing phone calls from patients. This role ensures that appropriate medical records, history, and patient status are available to the provider. The nurse will provide professional nursing assessments, prioritize treatments according to the urgency of need, and initiate appropriate care in a timely manner.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

1) Assessment & Triage: Collects subjective and objective data over the telephone or in-person. Identifies high-risk conditions and identifies urgency of patient need based on Call Distribution Protocols. Communicate data to Physician or Provider and carries out orders efficiently.

2) Documentation: Documents in the Electronic Medical Record (EMR) all conversations, changes in patient conditions, medications, and plans of care in a concise and logical manner. Responsible for gathering all records and results for emergent triage appointments.

3) Patient Education & Advocacy: Provides verbal and written education to patients/families regarding their condition and plan of care. Acts as a patient advocate for UroHealth Partners, seeking appropriate guidance and demonstrating a respectful rapport with all staff and patients.

4) Clinical Support: Performs medication prior authorizations, refill requests, patient requested paperwork, and reviews imaging, labs, and pathology reports. Acts as a liaison between the provider and the patient to relay results and recommendations.

5) Professionalism: Takes all hospital consults and Physician-to-Physician calls. Must work as a team member and perform actions consistent with established policies and procedures, including standards
