Wound Care Nurse Practitioner / Physician Assistant

Join a growing, mission-driven clinical team bringing specialized wound care directly to patients across skilled nursing facilities, assisted living, memory care, independent living, inpatient rehab, and inpatient psychiatric hospitals. This is hands-on, high-impact medicine where your clinical judgment directly improves healing outcomes, prevents complications, and restores quality of life for patients who need it most.
What You'll Do
Evaluate, treat, and manage acute and chronic wounds per current standards of care
Develop and implement individualized wound care treatment plans
Educate facility staff on wound management best practices
Address primary care concerns and determine appropriate medical urgency
Order and interpret diagnostic labs and imaging as needed
Consult with specialty providers when appropriate
Prescribe medications within your scope of practice
Collaborate with supervising physicians and interdisciplinary care teams
Maintain accurate, timely documentation and billing
Apply strong clinical judgment across routine and complex cases, and across multiple facility types and patient populations
What You Bring
Active Wisconsin APRN or Physician Assistant license
Wound Care Certification (or willingness to obtain within 6 months of hire * We will pay for your certification*)
Active BLS certification
DEA registration (or eligibility to obtain)
Strong clinical assessment and wound management skills
Comfort working independently across multiple sites
Excellent communication and patient education skills
Strong critical thinking and problem-solving ability
A commitment to high-quality, patient-centered care
