About This Position

Oakwood Village has an exciting opportunity for qualified candidates to join our Oakwood Village senior living community facility team as a Licensed Practical Nurse in our Skilled Nursing Facility. The LPN position is responsible for contributing to the assessment, planning, implementation and evaluation of resident care on a defined unit in the skilled nursing.

The LPN collaborates with nurses from all shifts, interdisciplinary staff, and physicians on issues related to the management of resident care. Responsibilities include direct resident and family care, mentorship of CNA staff and collaborating with registered nurses.

The LPN position reports to the Director of Nursing.

ESSENTIAL RESPONSIBILITIES of a LPN (not meant to be an all inclusive list)

Performs resident care including but not limited to: medication administration, completion of treatments, data collection, and basic teaching, and reinforcement education.

Contributes to the assessment, planning, implementation, evaluation, and documentation of resident conditions and care.

Documents and reports changes in condition, intervention, and resident response in coordination with RN. Initiates/follows-up/discontinues acute care plans as appropriate.

Updates Care Plan consulting with RN as appropriate.

Communicates Care Plan changes with Nursing and therapy staff .

Attends Care conferences as designated.

Reports changes in resident condition to supervisor and seeks guidance from RN as appropriate.

Delivers nursing care in compliance with Nurse Practice Act, State and Federal regulations applicable to SNF setting and policies and procedures of Oakwood Village.

Collaborates with nursing team, medical consultants, and other interdisciplinary team members to ensure resident and family needs are met.

Demonstrates effective time management and problem solving skills.

Communicates to physicians with any changes in condition, needs for order clarification, and when necessary transcription of physician orders, including EMR entry, per Oakwood policy.

ESSENTIONAL QUALIFICATIONS of a LPN

Knowledge of HSS 132 and all Federal tags preferred.

Knowledge of Medicare A requirements preferred.

Effectively works in a partnership environment which fosters effective team work in meeting the mission of Oakwood.

Independent initiation based on sound judgment.

Compliance with Oakwood's Nursing dress code policy.

EXPERIENCE AND/OR TRAINING

Licensed practical nurse license required.

One year relevant nursing experience in long term care preferred

CPR Certified required
