Clinical Services Supervisor Responsibilities
As the Clinical Services Supervisor – LPN, you will oversee the clinical operations of our assisted living community and provide leadership, direction, and clinical support to our resident care team.
Resident Care & Clinical Leadership
Oversee the delivery of high-quality resident care in accordance with individualized resident needs and care plans.
Provide hands-on clinical support and guidance to caregivers and medication administration staff.
Serve as a clinical resource for residents, families, caregivers, and leadership.
Complete resident assessments and evaluate resident needs before admission and throughout residency.
Monitor changes in resident condition and coordinate appropriate interventions and services.
Participate in and lead Individual Service Plan (ISP) meetings.
Coordinate resident services with physicians, pharmacies, home health agencies, hospice, therapists, hospitals, and other healthcare providers.
Promote resident dignity, independence, safety, and quality of life.
Staff Leadership & Supervision
Provide visible, proactive leadership to the clinical services team.
Supervise, coach, mentor, and develop direct care and medication administration staff.
Assist with staff training, competency validation, and ongoing clinical education.
Support staff in following resident care plans, medication procedures, and clinical policies.
Promote accountability while maintaining a positive and supportive workplace culture.
Medication & Clinical Compliance
Oversee medication management and medication administration processes.
Monitor medication documentation and identify opportunities for improvement.
Participate in medication audits and quality assurance activities.
Ensure clinical practices comply with applicable Wisconsin DHS 83, Chapter 50, and other state and federal requirements.
Maintain current knowledge of Wisconsin assisted living regulations and clinical best practices.
What We're Looking For
Required Qualifications
Current Wisconsin LPN license in good standing
Minimum 3 years of professional nursing experience, preferably in assisted living, long-term care, memory care, skilled nursing, or another senior care setting
Strong clinical assessment and communication skills
Ability to lead and supervise a healthcare team
Strong organizational and time-management skills
Ability to work independently while collaborating with an interdisciplinary team
Strong understanding of resident rights, documentation, medication management, and clinical compliance
Must meet applicable Wisconsin DHS 83 administrator/supervisory requirements, including minimum age requirements
Preferred Qualifications
Previous supervisory or leadership experience
Experience in CBRF, assisted living, memory care, long-term care, or skilled nursing
Knowledge of Wisconsin DHS 83
Experience with medication management and medication administration
Experience with ISP/care planning
Experience with staff training and competency development
Experience working with hospice, home health, physicians, pharmacies, and other healthcare partners
