About This Position
Job description:
The Director of Admissions / Community Liaison serves as the primary referral development and admissions leader for the Skilled Nursing Facility (SNF). This position is responsible for building and maintaining relationships with hospitals, physicians, assisted living facilities, home health agencies, case managers, discharge planners, and other referral sources while coordinating admissions and ensuring appropriate clinical and financial placement of residents.
The Director of Admissions / Community Liaison utilizes clinical knowledge as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to evaluate referrals, facilitate smooth transitions of care, and promote the facility's services within the community. This role is critical to maintaining census growth, resident satisfaction, and positive community partnerships.
The Director of Admissions/Community Liaison is considered a nurse leader within the organization and may be required to participate in the facility's administrative and nursing leadership on-call rotation as determined by operational needs.
Qualifications
Required
Current unrestricted LPN or RN license in the state of employment.
Minimum of 2 years of nursing experience in skilled nursing, long-term care, rehabilitation, acute care, or related healthcare setting.
Strong understanding of post-acute care services and healthcare reimbursement systems.
Excellent communication, organizational, and interpersonal skills.
Ability to build relationships with healthcare professionals and community partners.
Valid driver's license and reliable transportation.
Preferred
Previous admissions, case management, discharge planning, liaison, business development, or marketing experience.
Knowledge of Medicare, Medicaid, Managed Care, and commercial insurance.
Experience with electronic medical records and healthcare CRM systems.
Bachelor's degree in Nursing or healthcare-related field.
Knowledge, Skills, and Abilities
Strong clinical assessment and critical-thinking skills.
Ability to review medical records and determine admission appropriateness.
Knowledge of skilled nursing regulations and post-acute care services.
Strong sales, networking, and relationship-building abilities.
Effective presentation and public speaking skills.
Ability to manage multiple priorities in a fast-paced environment.
Proficiency in Microsoft Office and healthcare software systems.
