Job Summary:
In this role, you will be responsible for providing direct nursing care to residents, supporting daily clinical operations, and collaborating with interdisciplinary team members to ensure high-quality, person-centered care in accordance with resident care-plans, federal, state, and facility guidelines. You will work under the direction of a Registered Nurse or physician.
Key Responsibilities:
Deliver safe, effective, and compassionate nursing care to residents in accordance with individual care plans.
Administer medications and treatments accurately and safely
Monitor and assess residents’ health conditions; promptly report changes to the RN or provider and family as needed
Document care, observations, and assessments in the electronic health record (EHR)
Provide guidance and supervision to care providers and RA’s
Communicate effectively with physicians, families, and other care team members
Participate in care plan meetings, quality improvement activities, and regulatory surveys
Uphold infection control standards and safety procedures
Other duties as assigned
Requirements:
High school diploma or GED required
Completion of an accredited practical nursing program
Active, unencumbered LPN license in the state of practice; compact license may be acceptable depending on state requirements
Prior supervisory experience preferred
Long-term care or similar floor experience preferred
Compassionate, patient, and reliable with strong communication skills
Understand, respect, and maintain resident confidentiality
Ability to lift, move, and assist residents safely and comfortably
Must pass required criminal background check in order to serve vulnerable adults
Must be screened TB and other state-required health care conditions
Be able to read, write, and comprehend English
Physical Demands and Work Environment:
Indoors, climate-controlled assisted living facility with a focus on personalized care
Requires physical activity including standing, walking, lifting, and assisting residents
Possible exposure to unpleasant odors.
Possible exposure to chemicals as indicated in the MSDS manual.
Continuous exposure to clients and/or family members who may be under stress.
