About the Role
At Meadowbrook, we pride ourselves on delivering high-quality care—and that includes skilled nursing and clinical support for our residents. As a Licensed Practical Nurse (LPN), you’ll play a critical role in providing direct resident care, administering medications, and collaborating with the healthcare team to ensure residents’ physical and emotional needs are met.

Key Responsibilities
Administer medications and treatments according to care plans and facility policies
Provide direct resident care, including vital signs, wound care, and daily assessments
Document resident care accurately in medical records
Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
Monitor resident conditions and report changes promptly
Support residents’ emotional well-being and comfort
Follow infection control and safety procedures consistently
Attend in-service trainings and professional development opportunities
Complete additional duties as assigned by the Nursing Director or Administrator
What We’re Looking For
Licensed Practical Nurse (LPN) in good standing
Compassionate and patient-centered
Strong communication and teamwork skills
Ability to prioritize tasks and work independently as needed
Dedication to providing high-quality resident care
