About This Position
We're seeking a Registered Nurse to support our dedicated Home Health team in an admissions and float capacity.
Shift: Per Diem
Our service area covers King County. In this rewarding position, the physical therapist works collaboratively with other Home Health team members to admit patients to service and develop a comprehensive plan of care that meets the needs of patients and their families. Care settings include seeing patients in their private residences, adult family homes, and assisted and independent living facilities.

Experience
Collaborate with healthcare professionals, Oasis (EHR software), Patient resource coordination, Home health care assistance, RN License, Initial assessment (medical evaluation), In-home healthcare experience, Care documentation, Driver's License, Multidisciplinary team collaboration for treatment planning, Working with families, Computer skills, 1 year, Patient data collection, Care coordination, Home health agency experience, Group home experience, Entry level, Physical therapy
