Job Overview
We are seeking a dedicated and dynamic Nurse Leader specializing in Memory Care to oversee clinical operations within our specialized memory care community at Eagle Crest North. The ideal candidate will possess strong leadership skills, extensive nursing experience, and a passion for enhancing the quality of life for individuals living with dementia and related cognitive impairments. This role offers an opportunity to lead a compassionate team, ensure adherence to regulatory standards, and promote best practices in memory care nursing. Along with the Program Manager this Nurse has primary responsibility for 18 memory care residents.
Duties
Provide clinical supervision and leadership to nursing staff, fostering a collaborative and patient-centered environment.
Develop, implement, and evaluate individualized care plans tailored to residents with dementia or other memory-related conditions.
Ensure compliance with all federal, state, and local regulations
Oversee medication management processes, including medication administration, delegation, documentation, and safety protocols.
Participate in individual staff training on memory care best practices, behavioral health management, and infection control procedures.
Monitor resident health status through assessments, vital signs, and physiological evaluations; respond promptly to acute changes or emergencies.
Promote a culture of continuous improvement by integrating evidence-based practices such as trauma-informed care and behavioral health strategies.
Collaborate with interdisciplinary teams to facilitate all care from admission to discharge or end of life, family communication, and community resource integration.
Qualifications
Eligible to practice nursing in the state of Wisconsin, ensuring proper and timely licensure if transferring states
Experience in long-term care settings, including memory care or dementia-specific units beneficial, but not required
Demonstrated leadership skills with prior supervisory or managerial roles in nursing environments beneficial but we will train.
Knowledge of advanced clinical procedures and sterile processing procedures for things such as ostomy care, wound care, heart health, diabetes management and dementia.
Familiarity with electronic health record systems for medical documentation and communication
Experience working with individuals with developmental disabilities or behavioral health needs is highly desirable.
Understanding of regulatory standards including DHS 83 and DHS 89 requirements and infection control protocols.
Excellent communication skills coupled with a compassionate approach to resident care and family engagement. Compensation:
RN Starting: $41-$45 + per hour
LPN Starting: $31-$35 + per hour
Pay: $41.00 - $45.00 per hour
Expected hours: 36.0 – 40.0 per week
