We are seeking a dynamic and compassionate Resident Care Manager to lead and coordinate resident care services within our long-term care facility. Responsibilities
Lead and supervise nursing staff and care teams to deliver exceptional resident-centered care aligned with facility policies and state healthcare regulations.
Develop, implement, and monitor individualized care plans that address residents’ medical, emotional, and social needs, including dementia care and behavioral health considerations.
Ensure accurate medical documentation using EMR (Electronic Medical Records) systems such as Epic or Cerner, maintaining compliance with HIPAA standards.
Oversee medication administration, catheter care, tube feeding, and other clinical interventions while adhering to safety protocols.
Conduct comprehensive patient assessments including vital signs, physical examinations, and diagnostic evaluations to inform care strategies.
Coordinate discharge planning, family communication, and interdisciplinary team collaboration to optimize resident outcomes.
Manage staffing schedules, supervise team members’ performance, and promote ongoing training in areas such as infection control, basic life support (BLS), and aseptic technique.
Qualifications
Valid nursing license with current state registration; RN (Registered Nurse) preferred.
Extensive experience working in long-term care facilities or nursing homes with a strong background in geriatrics and dementia care.
Proficiency in EMR/EHR systems such as Epic, Athenahealth or eClinicalWorks; familiarity with ICD-9/10 coding and utilization review processes.
Demonstrated leadership experience managing nursing teams or care facility staff; supervisory skills required.
Knowledge of state healthcare regulations, long-term care regulations, HIPAA compliance, and NCQA standards.
Critical care experience including ICU or Level I/II Trauma Center exposure highly desirable.
Strong understanding of medical terminology, anatomy knowledge, pharmacology principles related to medication administration and injections.
