About This Position

Overview

Join our dedicated healthcare team as a Certified Nursing Assistant (CNA) in a long-term care (LTC) setting, where your compassionate care and clinical skills will make a meaningful difference in residents’ lives. This vital role involves providing direct patient care, supporting daily activities, and ensuring a safe, comfortable environment for individuals with diverse needs, including geriatrics, memory care, and individuals with disabilities. Responsibilities

Deliver high-quality personal care to residents, including assistance with bathing, dressing, grooming, and mobility using equipment such as Hoyer lifts and other patient transfer methods.

Monitor and record vital signs, observe changes in patient condition, and report findings accurately using electronic health record (EHR) systems like Epic or Cerner.

Administer medications and treatments following physician orders while maintaining strict adherence to HIPAA regulations and patient confidentiality standards.

Support residents with activities of daily living (ADLs), including feeding, toileting, and mobility assistance, ensuring safety through proper manual handling and safe lifting practices.

Assist with specialized care needs such as Alzheimer’s/dementia care, behavioral health management, hospice support, or trauma-related care within a Level I or Level II trauma center environment.

Maintain a clean and safe environment by following aseptic techniques during procedures like catheter care or wound management.

Collaborate effectively with interdisciplinary teams including nursing staff, physicians, therapists, and family members to coordinate comprehensive patient care plans.

Skills

Proven experience working with geriatric patients in long-term care facilities or nursing homes; familiarity with memory care and dementia-specific protocols preferred.

Strong knowledge of medical terminology, anatomy, physiology, and basic life support (BLS), including CPR certification.

Skilled in using electronic health records (EHR/Epic/eClinicalWorks) for medical documentation and patient data management.

Experience working with individuals with developmental disabilities or behavioral health needs is highly desirable.

Ability to perform heavy lifting safely using proper techniques; manual handling skills essential for patient transfers with equipment like Hoyer lifts.

Excellent caregiver communication skills to effectively interact with patients of all ages—including pediatrics—and their families.

Knowledge of infection control practices applicable in hospital medicine, outpatient clinics, or urgent/emergency care settings; familiarity with infectious disease protocols is advantageous.

Capable of working in fast-paced environments such as ICU or emergency medicine units; experience in acute or sub-acute care settings is a plus.
