Schedule: 5x8 | 40 hours/week
Shift: Day’s
Pay Rate: $2300-$2500/week
Key Responsibilities:
· Provide skilled nursing care to patients in their homes.
· Conduct comprehensive patient assessments and develop individualized plans of care.
· Perform Start of Care, Resumption of Care, recertification, discharge, and routine visits.
· Monitor patient conditions and communicate changes to physicians and the interdisciplinary care team.
· Educate patients and caregivers on medications, treatments, disease management, and safety.
· Coordinate patient care and ensure compliance with home health standards and regulatory requirements.
· Complete accurate and timely clinical documentation in HomeCare HomeBase (HCHB).
· Meet the required minimum productivity of 30 points per week.
· Travel throughout Lubbock and surrounding service areas, including other territories within approximately a 60-mile radius, as needed.
Requirements:
· Active Registered Nurse (RN) license.
· Recent Home Health experience, with experience from 2024–2025 required.
· HomeCare HomeBase (HCHB) experience required.
· Strong clinical assessment, documentation, communication, and organizational skills.
· Ability to work independently and manage a home health caseload effectively.
· Valid driver's license and reliable transportation.
· Ability and willingness to travel throughout the assigned service area.
