Job Overview
We are seeking a compassionate, dependable, and energetic Care Manager to join our Assisted Living and Memory Care team. In this role, you will provide hands-on support to residents by assisting with activities of daily living (ADLs), promoting independence, ensuring resident safety, and providing quality care that enhances each resident’s overall well-being.
The ideal candidate is a team player who is passionate about caring for seniors, communicates effectively with residents, families, and coworkers, and takes pride in providing respectful, person-centered care in a supportive community environment.
Responsibilities
Provide assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and personal hygiene.
Support residents with medication reminders and other care needs according to their individualized service plans.
Assist with resident transfers, ambulation, and safety needs while following proper care procedures.
Provide compassionate support to residents living with memory loss and dementia-related conditions.
Monitor and report changes in residents’ physical, emotional, and behavioral conditions to the appropriate team members.
Complete accurate and timely documentation, including daily charting, care notes, and resident records.
Follow individualized care plans and communicate any changes or concerns regarding resident needs.
Assist with meal support, hydration, and maintaining resident comfort and dignity.
Respond promptly to resident requests, call lights, and care needs.
Work collaboratively with nursing, leadership, activities, dining, and other team members to provide excellent resident care.
Maintain a clean, safe, and welcoming environment for residents.
Build positive relationships with residents and families through respectful and compassionate communication.
Requirements
Previous experience working with seniors in Assisted Living, Memory Care, skilled nursing, or a similar healthcare setting preferred.
Experience assisting with ADLs and providing personal care support.
Ability to accurately complete documentation and use electronic charting systems.
Strong communication skills and the ability to work effectively as part of a team.
Compassionate, patient, and respectful approach when caring for older adults.
Ability to recognize and report changes in resident condition.
Must be reliable, dependable, and committed to providing high-quality care.
current CNA or HCA license
