Position: Psychiatric Mental Health Nurse Practitioner (PMHNP) – Post-Acute / LTC
Location: Partner facilities throughout Nebraska.
Elite Professional Providers, LLC is seeking a high-caliber Psychiatric Nurse Practitioner to oversee behavior health and psychiatric therapies across our partner facilities in Nebraska.
This role blends the flexibility of telemedicine with strategic in-person rounding.
Nebraska law recognizes Full Practice Authority once an APRN completes a mandatory transition-to-practice period. This position accommodates both fully independent practitioners and those utilizing our built-in clinical network to complete transition milestones. You will manage panels across:
Skilled Nursing Facilities (SNF)
Long-Term Care (LTC) Facilities
Assisted Living Facilities (ALF)
Residential Care Facilities (RCF)
Memory Care Facilities
Key Responsibilities
Specialized Psychiatric Care Delivery
Clinical Evaluations: Perform thorough psychiatric intake evaluations, risk assessments, and establish primary diagnostic profiles under DSM-5-TR standards.
Geriatric Psychiatry Management: Address complex diagnostic scenarios, including late-onset psychosis, major depressive disorders, and aggressive behaviors linked to advanced dementia.
Crisis Prevention: Formulate crisis mitigation strategies to minimize emergency psychiatric hospital transfers from post-acute care environments.
2. Nebraska Prescriptive Authority & Compliance
Prescriptive Practices: Formulate and manage pharmacologic care plans including Schedule II-V controlled substances in compliance with the Nebraska Department of Health and Human Services (DHHS).
Transition Alignment: For providers who have completed their 2,000-hour practice transition, practice fully independently. For newer graduates, we provide structured, remote oversight to sign off on transition requirements seamlessly.
PDMP Inspections: Review the Nebraska Prescription Drug Monitoring Program to track and safely manage controlled substance therapies.
3. Multi-Facility Rounds & Workflow
Rounding to Skilled/ Long Term Care Facilities, Assisted Living facilities, Residential Care Facilities and Memory Care Facilities.
Collaborative Quality Panels: Work alongside facility medical directors and attending primary care providers to execute compliant gradual dose reductions (GDR).
Practice Workflow & Administrative Relief
Administrative Teams: Background operations, document signing chains, and prior authorizations are fully executed by our office support staff.
Primary Care Integration: Medical and wound-care needs are fully managed by separate on-site provider panels, leaving you free to focus purely on behavioral health.
Zero On-Call Burden: No mandatory night or weekend on-call shifts; full triage coverage is handled by our staff.
Pay Models & Compensation Options
Visit Revenue Model: Receive 55% of collected insurance claims for all physical or telemedicine visits.
Tax Architecture: Highly customizable contract structures tailored to your practice goals: choose between full 1099, W2 with benefits, or a hybrid layout.
Professional Benefits & Practice Allowances
Malpractice Coverage: 100% company-paid professional liability coverage ($1,000,000 per claim / $3,000,000 aggregate).
Practice Benefits: Annual allowance up to $3,500 plus 5 paid days off for CME, scaled to your active building panel count. (Dependent on FT / PT Status)
Licensure Protection: Full corporate financial coverage for your Nebraska state licenses, controlled substance registrations, and DEA certificates.
Qualifications
Master's or Doctoral degree in Nursing with a specialized PMHNP focus.
Active, unrestricted APRN license issued by the Nebraska DHHS.
National Board Certification (ANCC) as a Psychiatric Mental Health Nurse Practitioner.
Active federal DEA registration located in Nebraska.
Valid driver's license and active auto insurance.
Pay: $150,000.00 - $250,000.00 per year
