Franciscan Healthcare is seeking a compassionate and dedicated Home Health and Hospice Aide to join our Home Health and Hospice team where every interaction is an opportunity to make a meaningful difference.
In this vital role, you’ll provide hands-on support to patients in the comfort of their own homes, working under the guidance of licensed medical professionals. You’ll play a key part in enhancing quality of life by assisting with personal care, promoting comfort and safety, supporting daily activities, and performing essential nursing tasks. Hours: Part-time; general hours are from 7:30am-4:30pm, Monday-Friday
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
Ensure quality and safe delivery of personal care services
Participate in the development and implementation of the plan of care
Implement current personal care services; discuss plan of care with RN
Provide patient care in regard to hygiene, mobility, comfort, psychosocial, spiritual and nutritional needs
Carry out treatments, collection of specimens, vital signs, intake and output, and monitoring and emptying drainage devices
QUALIFICATIONS:
Education and/or Experience
Nursing Assistant or Hospice Aide certification through an approved program
Experience in hospice or related health care field preferred
Valid driver’s license
Other Competencies
Interpersonal skills requiring courtesy and tact are necessary to communicate effectively
Medium strength with lifting of 50 lbs. occasionally, 25lbs. frequently, and 10 lbs. constantly (Lifting over 35 lbs requires assistive devices be used)
Speak and understand the English language
Ability to properly lift position and transport patients (with assistance as defined)
Ability to stabilize unsteady patient when ambulating
Ability to record information accurately and follow instructions exactly
