Job Overview
The LPN/Client Care Coordinator is responsible for coordinating and overseeing client care to ensure safe, compassionate, and high-quality homecare services. This role serves as a liaison between clients, families, caregivers, and the agency, ensuring care plans are developed, implemented, and adjusted to meet each client’s physical, emotional, and social needs while maintaining compliance with agency policies and regulatory standards.
1. Operational Leadership:
Develop and implement strategic plans and objectives to achieve business growth and operational excellence.
Oversee day-to-day operations, including but not limited to: staffing, answering phone calls, scheduling, client services/calls, and caregiver management.
Ensure compliance with all relevant regulations, policies, and procedures.
Maintain an outstanding relationship with Veterans Administration, State Funded Programs and all other relevant organizations.
Maintain regular and predictable attendance in order to maintain office hours.
Maintain a professional appearance to Visiting Angels standards
2. Client Care and Satisfaction:
Maintain a high standard of care delivery, ensuring the well-being and satisfaction of clients.
Assist with conducting client check in’s, intakes/reassessments and assessments if needed, development of customized care plans, and monitor service quality.
Address client concerns or issues promptly and effectively.
Make weekly calls to check in with clients to check in on services provided.
3. Staff Management and Development:
Train in office and in the field to Visiting Angels standards ensuring adequate staffing and maintaining agency efficiency.
Provide ongoing support, coaching, and performance feedback to office staff and caregivers to enhance their skills and professionalism.
Foster a positive and work environment that promotes teamwork, communication, and employee engagement.
4. Business Development and Marketing:
Assist in identifying opportunities for business growth and expansion in the local market.
Develop and oversee execution of marketing strategies to attract new clients and maintain relationships with referral sources.
Participate in community outreach activities, marketing and networking events to raise awareness of Visiting Angels services.
Requirements:
Current or former LPN/RN
1-2 Years Experience with Management preferred
Must be able to pass background check
Compensation:
$28/hr-$32/hr: 40hr weekly salary plus on-call rotation (On-Call paid out in overtime)
Bonus - $0.15/hr over determined base hours are established when 4 months training is completed. (or earlier depending on performance)
