About This Position

Job Overview

We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring compliance with federal and state regulations while facilitating the assessment and care planning processes for residents in a nursing home or hospital setting. This position requires a strong understanding of medical terminology, case management, and discharge planning, as well as experience with MDS (Minimum Data Set) assessments.

Duties

Conduct comprehensive assessments of residents using the MDS tool to ensure accurate documentation of their needs and care plans.

Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the standards of managed care.

Maintain up-to-date knowledge of regulations and guidelines related to MDS assessments and documentation.

Utilize PCC or similar electronic health record systems for data entry and management.

Provide training and support to nursing staff on MDS processes, documentation standards, and best practices.

Monitor resident progress and coordinate with primary care providers to ensure continuity of care during discharge planning.

Participate in quality assurance initiatives to improve patient outcomes and compliance with healthcare standards.

Qualifications

Previous experience in a hospital or nursing home setting is preferred.

Strong knowledge of medical terminology and familiarity with case management principles.

Proficiency in using electronic health record systems, particularly PCC, is a plus.

Understanding of managed care processes and regulations related to MDS assessments.

Excellent communication skills, both verbal and written, with the ability to collaborate effectively with healthcare professionals.

Detail-oriented with strong organizational skills to manage multiple assessments and documentation requirements efficiently.

Certification in MDS coordination or related fields is advantageous but not mandatory.

Job Type: Full-time

Pay: $45.00 - $60.00 per hour
