About This Position
Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will help ensure patients receive personalized, efficient, and safe nursing services. Responsibilities
Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
Support patients with activities of daily living and specialized care needs such as dementia, Alzheimer’s, memory care, and hospice services.
Manage airway care, catheterization, tube feeding, wound dressing changes, and ventilator support as required.
Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices.
Ensure compliance with HIPAA confidentiality policies and maintain meticulous medical documentation for legal and clinical purposes.
Skills
Proven experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring acute or chronic care.
Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and electronic health records (EMR/EHR systems) such as Epic or Cerner.
Skilled in performing triage assessments, first aid procedures, venipuncture (phlebotomy), catheter care, and sterile techniques.
Experience in specialized areas such as ICU or Level I/II trauma centers, emergency medicine, or hospital medicine is highly desirable.
Ability to communicate effectively with patients, families, caregivers, and healthcare teams to ensure optimal patient outcomes.
Knowledge of infection control protocols and long-term care regulations to promote safe practices across all settings.
Certification in basic life support (BLS), CPR, and other relevant nursing credentials required.

Pay: $45.00 - $53.00 per hour
