About This Position

Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will help ensure patients receive personalized, efficient, and safe nursing services. Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
  • Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
  • Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
  • Support patients with activities of daily living and specialized care needs such as dementia, Alzheimer’s, memory care, and hospice services.
  • Manage airway care, catheterization, tube feeding, wound dressing changes, and ventilator support as required.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices.
  • Ensure compliance with HIPAA confidentiality policies and maintain meticulous medical documentation for legal and clinical purposes.

    Skills

  • Proven experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring acute or chronic care.
  • Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and electronic health records (EMR/EHR systems) such as Epic or Cerner.
  • Skilled in performing triage assessments, first aid procedures, venipuncture (phlebotomy), catheter care, and sterile techniques.
  • Experience in specialized areas such as ICU or Level I/II trauma centers, emergency medicine, or hospital medicine is highly desirable.
  • Ability to communicate effectively with patients, families, caregivers, and healthcare teams to ensure optimal patient outcomes.
  • Knowledge of infection control protocols and long-term care regulations to promote safe practices across all settings.
  • Certification in basic life support (BLS), CPR, and other relevant nursing credentials required.

    Pay: $45.00 - $53.00 per hour
