Job Overview
We are seeking a dedicated and highly skilled Licensed Practical Nurse (LPN) to provide compassionate, comprehensive patient care across diverse healthcare settings. The ideal candidate will possess a strong foundation in nursing practices, with experience in acute, long-term, and community-based care environments. As an integral part of our healthcare team, the Licensed Practical Nurse will deliver high-quality patient assessments, administer treatments, and support overall patient well-being while adhering to regulatory standards and organizational policies. Duties
Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
Administer medications and injections accurately, including IV infusions and other specialized treatments, following established protocols and safety guidelines.
Assist with medical procedures such as catheterization, wound suturing, tube feeding, and specimen collection/processing while maintaining aseptic techniques.
Support patients with complex needs including dementia care, Alzheimer's care, memory care, and behavioral health management.
Provide emergency response and triage services in urgent care or emergency room settings, utilizing skills in airway management and acute pain management.
Collaborate with multidisciplinary teams to develop personalized care plans for patients in settings such as nursing homes, assisted living facilities, hospice care, or hospital units including ICU and Level I Trauma Centers.
Maintain accurate medical documentation compliant with HIPAA regulations and ensure proper coding using ICD-9/ICD-10 standards for diagnoses and CPT coding for procedures.
Educate patients and their families on health coaching topics such as medication administration, nutrition, discharge planning, and chronic disease management.
Ensure infection control protocols are strictly followed to prevent cross-contamination in all clinical environments.
Participate in ongoing training related to medical imaging procedures, cardiology (including cardiac catheterization), or specialized areas like ophthalmology or dermatology when applicable.
Assist with complex cases involving ventilator management, dialysis procedures, or post-acute care transitions to optimize patient outcomes.
Adhere to all safety standards including basic life support (BLS), sterile processing techniques, and proper use of medical equipment such as Hoyer lifts or neonatal ventilators.
Requirements
Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in good standing.
Proven experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, assisted living facilities, or home & community care settings.
Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for documentation and order entry.
Strong knowledge of physiology, anatomy, pharmacology (including medication administration), and medical terminology relevant to nursing practice.
Experience with IV insertion/infusion therapy and phlebotomy techniques essential for patient treatment plans.
Familiarity with ICD-9/ICD-10 coding standards for accurate diagnosis documentation; CPT coding knowledge is a plus.
Ability to perform physical examinations and diagnostic evaluations effectively while maintaining patient comfort and safety.
Excellent communication skills to collaborate effectively with patients of all ages—including pediatrics (children/toddlers) and neonatal units—and multidisciplinary teams.
Knowledge of infection control practices and aseptic techniques required for sterile procedures such as suturing or spinal taps.
Experience working with individuals with disabilities or developmental challenges is highly desirable.
Certification in basic life support (BLS) is mandatory; additional certifications such as ACLS or PALS are advantageous depending on the work setting.
