We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our compassionate healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments.

Responsibilities
Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans.
1 on 1 patient care, Follow up with MD Orders
Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations.
Assist with medical procedures such as sterile processing, specimen collection, tube feeding, and airway management.
Maintain detailed medical documentation within Electronic Health Records (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations.
Provide specialized care for patients with complex needs such as acute pain management, infectious disease care, hospice support, and trauma-related treatments in Level I or Level II trauma centers.
Collaborate effectively with interdisciplinary teams on case management, diagnostic evaluations, and patient education to optimize health outcomes.
Support patients across various settings including inpatient units like ICU or PICU, outpatient clinics such as urgent or primary care, and community-based home care services.Experience
Valid licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
Proven experience working in diverse healthcare environments such as hospitals (including emergency room and trauma centers), skilled nursing facilities, hospice programs, or outpatient clinics.
Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; experience with ICD coding (ICD-9/10), CPT coding is a plus.
Strong background in specialized areas including geriatrics, dementia or Alzheimer's care, pediatric nursing (including toddler and infant care), and long-term care regulations.
Hands-on experience with IV infusion therapy, catheterization techniques, first aid procedures, and infection control practices.
Familiarity with medical terminology related to internal medicine, surgical procedures, respiratory therapy (ventilators), and diagnostic evaluation methods.
