About This Position
Most remote evening and overnight LPN jobs are glorified call centers. This one isn't.
At CareSola, you'll manage a dedicated panel of Medicare patients — the same people, every month. You'll know their names, their conditions, their families, and their goals. When a patient's blood pressure spikes, you're the one who catches it. When someone can't afford their medication, you're the one who finds a solution.
You're not answering random calls. You're a consistent, trusted presence in your patients' lives.
We're currently hiring for two shifts:
Evening Shift: 5:00 PM – 1:30 AM (30-minute break)
Overnight Shift: 12:00 AM – 8:30 AM (30-minute break)
Both are Monday–Friday.
What You'll Do
Manage a dedicated panel of Medicare patients through outbound calls
Review RPM biometric data (blood pressure, glucose, weight) and flag abnormal readings
Coordinate care across four Medicare programs: CCM, RPM, Principal Illness Navigation (PIN), and Community Health Integration (CHI)
Update care plans in the EHR; coordinate with physicians, pharmacies, and specialists
Connect patients with community resources for social needs (food, transportation, housing)
Document all patient interactions with detailed, billable time logs
Requirements
Active NLC multistate compact license (LPN/LVN) required
Must reside in one of these states: TX, FL, TN, AZ, UT, NC, SC
Must hold primary state of residence in an NLC member state
At least 1 year of clinical experience in any setting
Comfortable making outbound phone calls and building rapport with elderly patients
Proficiency with computers, EHR systems, and basic documentation tools
Reliable internet (25 Mbps download minimum) and a quiet, HIPAA-compliant home workspace
Computer with 8GB RAM, i5/Ryzen 5 or equivalent, SSD storage, and USB wired headset
Ability to reliably work the assigned overnight or evening schedule
Preferred
Prior experience with CCM (Chronic Care Management) and RPM (Remote Patient Monitoring)
Background in chronic care management, remote patient monitoring, home health, or care coordination
Bilingual English/Spanish
Familiarity with Medicare billing codes (99490, 99457, G0023, G0019)
Experience working remotely, especially on an evening or overnight schedule

Compensation & Details
$20–$25/hour based on experience
Full-time, 40 hrs/week, Monday–Friday
W2 employment, non-exempt; paid biweekly
100% remote — equipment not provided
Health insurance benefits are not currently available; this is an ACA marketplace-eligible W2 position
Training program (no prior CCM/RPM experience required — we will teach you)
