Job Summary

We are seeking a dynamic and dedicated Registered Nurse Case Manager to join our compassionate team. In this vital role, you will coordinate patient care plans, assess individual needs, and collaborate with healthcare professionals to ensure optimal health outcomes. Your expertise will help guide patients through complex medical journeys, providing both clinical oversight and personalized support.

Duties

· Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses.

· Provides professional nursing care by utilizing all elements of the nursing process.

· Assesses and evaluates the patient's status

· Uses health assessment data to determine nursing diagnosis.

· Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.

· Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician and the physician's plan of care.

· Counsels the patient and family in meeting nursing and related needs.

· Provides health care instructions to the patient as appropriate per assessment and plan.

· Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.

· Acts as a case manager when assigned by the Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for the assigned caseload.

Additional Duties

Actively participates in quality assessment performance improvement teams and activities.

Requirements

· Graduate of a National League for Nursing accredited school of nursing.

· Maintains a current CPR certification.

· Current valid driver’s license.

· Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

· Self-directed and able to work with minimal supervision.

· Responsible for supervising hospice aides and other LVNs on their caseload.

· Demonstrates excellent observation, problem-solving, verbal and written communication, and nursing skills per competency checklist.

· Shows ability to organize and prioritize workload independently.

· Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing services.

Pay: $45.00 - $55.00 per hour
