PM Shift | 2:00 PM – 10:30 PM with Every Other Weekend Rotation

CLINICAL SETTING
This opportunity provides experience across multiple levels of post-acute and residential care, including:
Short-Term Rehabilitation
Skilled Nursing / Long-Term Care
Community-Based Residential Care (CBRF)
RNs must be comfortable floating between assigned areas based on resident census and clinical needs.
KEY RESPONSIBILITIES
Provide comprehensive nursing care to short-term rehabilitation, LTC, and CBRF residents with varying levels of acuity and clinical needs.
Perform resident assessments, administer medications and treatments, provide wound and skin care, and promptly identify and report changes in condition.
Coordinate care with physicians, CNAs, rehabilitation therapy, pharmacy, residents, families, and interdisciplinary team members.
Manage admissions, transfers, physician orders, care plan implementation, and clinical documentation while maintaining resident safety and continuity of care.
Provide clinical direction and delegation to nursing assistants while ensuring compliance with infection prevention, resident safety, and facility standards.
REQUIREMENTS
Active Wisconsin or Compact RN License – REQUIRED
Minimum 2+ years of Skilled Nursing/Long-Term Care experience
Strong medication administration and resident assessment skills
Ability and willingness to float between Rehab, LTC, and CBRF
Previous Skilled Nursing Facility experience strongly preferred
ECS experience preferred but NOT required — training available
