Responsibilities
Assess patient needs and coordinate case management plans for stop-work scenarios, ensuring timely interventions and appropriate level of care.
Collaborate with physicians, nurses, social workers, and care teams to optimize discharge planning and post-acute care transitions.
Conduct thorough patient assessments, document progress in EMR/EHR, and monitor outcome metrics aligned with health regulations.
Facilitate utilization management, resource coordination, and compliance with managed care standards and hospital policies.
Communicate effectively with patients and families to explain care plans, responsibilities, and expected timelines.
Ensure patient safety and quality of care through ongoing evaluation and interdisciplinary collaboration.
Pay: From $110.00 per hour
