Job Summary
A Premier Senior Living Company is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality resident care for AL/ and Memory Care. Duties
Support residents with specialized needs including dementia care, Alzheimer’s care, memory care, behavioral health support.
Collaborate with interdisciplinary teams to develop and implement personalized care plans aligned with long-term care regulations and best practices.
Maintain meticulous medical records using EMR/EHR systems like Epic, Cerner, or Athenahealth while ensuring compliance with HIPAA confidentiality policies.
Provide patient education on health coaching topics such as medication management, nutrition, discharge planning, and post-acute care instructions.
Respond promptly to urgent situations including acute pain management, triage assessments in emergency or urgent care settings, and infectious disease control protocols.
Experience
Proven experience working as an LPN in Assisted Living and Memory care communities.
Demonstrated proficiency with EMR/EHR systems for accurate medical documentation and record keeping.
Background working with geriatric populations in geriatrics or memory care units; experience caring for individuals with dementia or Alzheimer’s disease is advantageous.
Current LPN license
Strong understanding of long-term care regulations along with experience working within skilled nursing facilities or assisted living communities.
