Your energetic approach and attention to detail will help foster a positive environment for patients of all ages, including those with complex medical needs such as dementia, memory care, or post-acute recovery. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring specialized care, including those in long-term care, assisted living, hospitals, and community-based settings.

Duties

Assist patients with activities of daily living such as bathing, dressing, feeding, and mobility using tools like Hoyer lifts and other assistive devices

Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature

Support medication administration under supervision, ensuring proper documentation in electronic health record (EHR) systems like Epic or Cerner

Maintain strict adherence to aseptic techniques and infection control protocols during patient care procedures

Provide compassionate dementia and Alzheimer’s care, memory support, and behavioral health assistance tailored to individual needs

Assist with catheter care, wound dressing changes, and other medical-surgical procedures as directed by nursing staff

Document patient information thoroughly in medical records while respecting HIPAA regulations and confidentiality standards

Collaborate with interdisciplinary teams in acute care environments such as ICU, emergency medicine, or Level I Trauma Centers to deliver comprehensive patient service

Support patients with developmental disabilities or behavioral health challenges in both inpatient and outpatient settings

Use EMR systems like eClinicalWorks or CareCloud to update patient data promptly and accurately

Requirements

Valid CNA certification recognized by the state or relevant licensing authority

Proven experience working with diverse patient populations including long-term care residents, pediatrics, geriatrics, or behavioral health patients

Prior experience in medical settings such as hospitals (including ICU or trauma centers), nursing homes, assisted living facilities, or home care is highly valued

Knowledge of medical terminology, anatomy, physiology, and aseptic techniques necessary for safe patient care

Ability to work effectively with people with disabilities or complex medical conditions such as infectious diseases or post-acute recovery cases

Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate documentation

Strong communication skills combined with a compassionate attitude toward patient service and caregiving responsibilities

Ability to stand for extended periods and perform physically demanding tasks using lifts or mobility aids when needed

Join our team if you’re passionate about providing exceptional patient-centered care! We are committed to supporting your professional growth through ongoing training opportunities in areas such as hospice care, pediatric nursing support (PICU), acute care procedures, and geriatrics.
