Resident Care Manager (RCM) - Shoreline, WASummary
We are seeking a dynamic and experienced Resident Care Manager (RCM) to join our leadership team in Shoreline, Washington. This key clinical leadership role is responsible for coordinating and overseeing resident care services, managing care plans, supporting nursing staff, ensuring regulatory compliance, and promoting exceptional resident outcomes. The ideal candidate is a compassionate Registered Nurse with strong clinical knowledge, organizational skills, and a passion for resident-centered care in a skilled nursing setting.
Essential Duties and Responsibilities
Coordinate and oversee resident care services for assigned units.
Lead and participate in resident assessments, care planning, and care plan meetings.
Ensure comprehensive and accurate MDS, care plans, and clinical documentation.
Monitor resident condition changes and coordinate appropriate interventions.
Collaborate with physicians, therapists, pharmacy, and interdisciplinary team members to ensure quality care.
Provide leadership, guidance, and clinical support to nursing staff.
Conduct rounds to evaluate quality of care, documentation, and resident satisfaction.
Manage admissions, transfers, and discharges to ensure smooth transitions of care.
Assist with infection prevention and control initiatives.
Participate in Quality Assurance and Performance Improvement (QAPI) activities.
Support survey readiness efforts and assist during state and federal inspections.
Investigate and resolve resident, family, and staff concerns in a timely manner.
Monitor and ensure compliance with federal, state, and facility regulations.
Assist with employee education, orientation, competency evaluations, and ongoing training.
Collaborate with the Director of Nursing and interdisciplinary team to achieve clinical and operational goals.
Perform additional duties as assigned.
RequirementsEducation and Experience
Graduate of an accredited nursing program.
Current Registered Nurse (RN) license in Washington State.
Minimum of two years of nursing experience in a skilled nursing, long-term care, rehabilitation, or related healthcare setting preferred.
Previous supervisory, charge nurse, or care management experience preferred.
Knowledge of MDS, care planning, and long-term care regulations preferred.
Certificates and Licenses
Active Washington State Registered Nurse (RN) license.
Current CPR certification preferred.
Other Qualifications
Strong clinical assessment and critical thinking skills.
Knowledge of state and federal regulations governing long-term care facilities.
Excellent communication, leadership, and organizational abilities.
Ability to work effectively with residents, families, physicians, and interdisciplinary teams.
Commitment to resident-centered care and quality outcomes.
Ability to manage multiple priorities in a fast-paced environment.
Strong documentation and computer skills.
Work Environment
Works in both office and resident care areas throughout the facility.
Frequent interaction with residents, families, staff, physicians, and regulatory agencies.
Subject to interruptions and changing priorities.
May require occasional weekend, holiday, or on-call support.
Exposure to healthcare-related environmental risks, including infectious diseases.
Physical Requirements
Ability to move throughout the facility during the workday.
Ability to stand, walk, bend, and reach as needed.
Ability to lift up to 25 pounds regularly and up to 50 pounds occasionally.
Ability to assist during emergency situations and resident evacuations when necessary.
Must be able to perform essential job functions with or without reasonable accommodation.
