Responsibilities
- Provide nursing care to patients in home health and hospice settings based on the established care plan

- Educate patients and families on care needs, symptom management, and safety
- Maintain clear, timely communication with the interdisciplinary team

- Experience providing skilled nursing care in home health and/or hospice settings
- Strong clinical assessment, documentation, and communication skills
