About the Role:
As an LPN at Lakeside Residence & Rehab, you will be responsible for providing personalized care to our patients, including administering medications, wound care, and assisting with daily living activities. You will work closely with our interdisciplinary team to develop and implement individualized care plans, while maintaining accurate records and communicating effectively with patients, families, and healthcare professionals.
Responsibilities:
Provide direct patient care, including administering medications, wound care, and assisting with daily living activities
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Maintain accurate and up-to-date records of patient care
Communicate effectively with patients, families, and healthcare professionals
Provide emotional support and encouragement to patients and families
Participate in ongoing education and training to stay current with industry advancements and best practices

Requirements:
Current LPN licensure in the State of Texas
High school diploma or equivalent required
Completion of an accredited LPN program
Minimum 1 year of experience as an LPN (TBD)
