About This Position

<Overview> Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) - Charge Nurse, where your expertise will lead and coordinate patient care in a fast-paced, compassionate environment. <Responsibilities>

  • Lead nursing teams by coordinating patient assessments, developing care plans, and ensuring adherence to clinical protocols.
  • Supervise daily operations within the unit or department, maintaining compliance with healthcare standards such as HIPAA and NCQA standards.
  • Oversee patient care activities including vital signs monitoring, medication administration (including injections and IV infusions), catheter care, and wound management.
  • Collaborate with interdisciplinary teams to facilitate discharge planning, case management, and post-acute care transitions.
  • Utilize EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth for accurate documentation of medical records, medical terminology entries, ICD coding (ICD-9/10), and medical documentation.
  • Provide mentorship and training to nursing staff on topics such as sterile techniques, airway management, infection control, and advanced procedures like spinal taps or gastrostomy tube feeding.
  • Ensure safety protocols are followed during emergency situations such as acute pain episodes or trauma responses in Level I or Level II trauma center environments.

    <Qualifications>

  • Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
  • Proven experience in critical care settings such as ICU, emergency room, or hospital medicine; experience in long-term care or assisted living is highly valued.
  • Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation procedures.
  • Hands-on experience with EMR/EHR systems like Epic or Cerner; familiarity with medical imaging reports and lab specimen processing is a plus.
  • Demonstrated ability to perform patient assessments including vital signs collection, physical examinations, venipuncture (phlebotomy), and tube feeding management.
  • Excellent communication skills with the ability to triage effectively and work with individuals with disabilities or those requiring dementia or Alzheimer's care.
  • Additional qualifications include IV insertion expertise, sterile processing knowledge, hospice or palliative care experience, and familiarity with infectious disease protocols.
