Duties
Deliver comprehensive nursing care to residents, including medication administration, wound care, and vital signs monitoring.
Perform detailed patient assessments to identify health needs and develop individualized care plans.
Document all patient interactions accurately in electronic health records (EHR) systems while maintaining strict compliance with HIPAA and confidentiality policies.
Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and ongoing treatment strategies.
Conduct triage assessments for incoming residents or visitors, prioritizing urgent needs and facilitating appropriate interventions.
Assist with specialized dementia and Alzheimer’s care, ensuring a safe environment that supports cognitive health and emotional well-being.
Skills
Proven experience working with geriatric patients in long-term care or skilled nursing facilities.
Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
Skilled in using EMR/EHR systems
Experience managing acute pain cases, hospice care protocols, and infectious disease control measures.
Ability to handle emergency situations confidently using basic life support (BLS) skills and airway management techniques.
Excellent communication skills for effective patient assessment, family education, and team collaboration.

Pay: $42.00 - $50.00 per hour
