About This Position
We are currently looking for an LPN or Certified Medical Assistant to join our Bismarck team as a Clinical Care Coordinator!
This position is responsible for assisting Nurse Practitioners care for patients in our clinic, as well as assisted living and home environments, making it a unique role beyond the traditional clinic environment. As a Clinical Care Coordinator, you will be an advocate for the patient's needs by collaborating with both the patient and their care team to ensure that the patient is supported throughout their healthcare journey.

Responsibilities:
Chronic Care Management (CCM) coordination and patient outreach
Preparing patient charts and ensuring records are organized and ready for provider review
Processing medication refill requests in accordance with provider protocols
Coordinating referrals, laboratory orders, and follow-up testing as directed by providers
Serving as the primary liaison between patients and providers, facilitating communication, and addressing patient questions and concerns
Conducting nurse visits within the scope of practice
Qualifications:
Active LPN license with the state of North Dakota OR graduate of a Medical Assistant program and nationally certified or registered through approved program (AAMA, CCMA, NHA, CMAC, AMCA, NCMA, NCCT)
Strong computer skills, including proficiency with EHR systems
Exceptional multitasking abilities and attention to detail
Ability to stay calm, organized, and efficient in a fast-moving environment
Excellent communication and interpersonal skills
Prior experience in healthcare, medical office, or similar role preferred, but not required
Offers of employment are contingent upon passing a background check and drug screen.
