About This Position

Job Overview We are seeking a dedicated and highly skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings including long-term care, hospitals, assisted living, and home health. As an LPN, you will play a vital role in providing compassionate, efficient nursing services, ensuring the well-being of our diverse patient population. Your expertise in medical documentation, medication administration, and patient assessment will contribute significantly to delivering high-quality healthcare services.

Duties

Perform comprehensive patient assessments by monitoring vital signs and conducting physical examinations to inform care plans.

Administer medications and injections accurately while adhering to prescribed protocols and safety standards.

Assist with procedures such as wound care, catheterization, ostomy care.

Maintain detailed and accurate medical records utilizing EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems in compliance with HIPAA regulations.

Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or those requiring memory care.

Educate patients and their families on health coaching topics including medication management, nutrition, and disease prevention strategies.

Support staff and participate in on-call nurse rotation.

Experience

Valid licensure as a Licensed Practical Nurse with current state certification.

Proven experience working in diverse clinical settings.

Demonstrated proficiency with EMR/EHR systems.

Background in caring for individuals with developmental disabilities or behavioral health needs is highly desirable.

Prior experience in long-term care facilities or assisted living communities emphasizing memory care and dementia support is preferred.

Knowledge of infection control practices and infectious disease care protocols to ensure patient safety across all environments.

Pay: From $30.00 per hour
