Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including skilled nursing facilities, primary care facilities, and assisted living environments. As an LPN, you will play a vital role in supporting patients' health and well-being while collaborating with registered nurses and other healthcare professionals.
Responsibilities
Administer medications and treatments as prescribed by physicians.
Monitor patients' vital signs and report any changes to the healthcare team.
Assist in the development and implementation of patient care plans.
Maintain accurate medical records using electronic health record systems such as PCC.
Perform IV insertions and other clinical procedures as required.
Educate patients and their families on health management and wellness practices.
Collaborate with multidisciplinary teams to ensure comprehensive patient care.
Admit and discharge patients.
Skills
Proficiency in using electronic health record systems, particularly PCC.
Ability to perform IV insertions competently and confidently.
Experience working in Skilled Nursing facility.
Excellent communication skills to effectively interact with patients, families, and healthcare teams.
Strong organizational skills with attention to detail in patient documentation and care plans.
