REGISTERED NURSE (RN) – ADMISSIONS | CLINICAL LEADER$40/HR WEEKENDS + MILEAGE | $35/HR WEEKDAYS + MILEAGE

Elite Professionals Home Care is looking for an experienced Home Health RN to join our team as an Admissions/Clinical Leader. If you’re confident in your clinical skills, passionate about patient care, and ready to make a difference from the very first visit, we want you!
THE SCHEDULE & PAY
$40/hour + mileage – Saturday & Sunday
Every other weekend minimum
Optional weekday hours: Monday/Thursday OR Tuesday/Friday
$35/hour + mileage for weekday visits
Typical hours: 8:00 AM–5:00 PM
WHAT YOU’LL DO
Complete OASIS admissions, assessments, recertifications, resumptions, and discharges
Develop individualized Plans of Care
Collaborate with physicians, therapists, aides, patients, and families
Provide clinical guidance and support to nursing staff
Perform skilled nursing assessments and patient visits
Utilize Homecare Homebase (HCHB) EMR
BRING YOUR WOUND CARE EXPERTISE
Wound care and wound vac experience are highly valued!
Already experienced? Excellent!

WHAT WE’RE LOOKING FOR
✔ Active Nebraska RN license
✔ Home health/home care experience required
✔ Strong clinical assessment and documentation skills
✔ Compassionate, confident, and dependable
✔ Wound care experience preferred
✔ Valid driver’s license, reliable transportation, and appropriate insurance
