Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. The ideal candidate will possess a strong foundation in patient care within long-term, assisted living, or nursing home settings, demonstrating expertise in managing the unique needs of geriatric and chronically ill populations. As an LPN in Long Term Care, you will play a vital role in delivering high-quality nursing services, supporting residents’ well-being, and ensuring compliance with healthcare regulations. Your proactive approach and clinical skills will contribute to creating a safe, respectful, and nurturing environment for our residents.
Duties
Provide comprehensive nursing care to residents, including medication administration, wound care, and vital sign monitoring.
Assist with activities of daily living such as bathing, dressing, and mobility support to promote independence and dignity.
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s needs.
Monitor residents’ health status through assessments and documentation using Electronic Health Record (EHR) systems.
Administer IV infusions and manage acute pain or symptom control as needed within scope of practice.
Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and safety standards.
Educate residents and their families on health conditions, medication management, and care procedures to foster understanding and engagement.
Requirements
Valid LPN license with current state licensure in good standing.
Prior experience working in long-term care facilities such as nursing homes or assisted living communities.
Demonstrated proficiency with EMR/EHR systems like Epic or Cerner for accurate medical documentation.
Knowledge of geriatric care principles, dementia care including Alzheimer’s management, and memory care practices.
Experience with IV insertion, catheterization, wound dressing changes, and basic life support (BLS).
Familiarity with infection control protocols, medication administration procedures, and long-term care regulations.
Strong communication skills to effectively coordinate with residents’ families, healthcare providers, and team members.
