Responsibilities:
Assist with transporting residents to and from medical appointments, therapy sessions, and social activities
Provide emotional support and companionship to residents during transportation
Monitor resident vital signs and report any changes to nursing staff
Assist with wheelchair and stretcher transfers
Maintain accurate records of transportation activities and resident vital signs
Collaborate with nursing staff to develop and implement individualized care plans
Participate in orientation and training programs to ensure compliance with facility policies and procedures

Requirements:
Current CNA certification
High school diploma or equivalent required
Ability to lift, transfer, and move residents as needed
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Valid driver's license
