About This Position
Licensed Practical Nurse (LPN) – Skilled Rehabilitation
We are currently seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join a Skilled Rehabilitation Unit providing care to patients recovering from hospitalization, surgery, illness, and injury.
This is a great opportunity for an LPN who enjoys hands-on clinical care, working as part of an interdisciplinary team, and helping patients progress toward their goal of returning home.
What You’ll Do
Provide direct nursing care to skilled rehabilitation and long-term care patients
Administer medications and treatments according to physician orders
Monitor patients and promptly communicate changes in condition
Complete accurate and timely clinical documentation
Provide wound care and other skilled nursing treatments within scope of practice
Coordinate with Nurse administration, physicians, therapy, CNAs, and other members of the interdisciplinary team
Communicate with patients and families regarding care and progress
Supervise and support CNAs during assigned shifts
Participate in care planning and help ensure individualized patient needs are met
Maintain compliance with facility policies and applicable state and federal requirements
What We’re Looking For
Current LPN license in good standing
Strong clinical assessment and communication skills
Dependable, organized, and team-oriented
Commitment to providing compassionate, resident-centered care
Skilled nursing, rehabilitation, post-acute, or long-term care experience preferred, but not required
New graduate LPNs are encouraged to apply
