Job Overview
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our CMS Five Star rated team! We are a secured 50 bed memory care facility, The ideal candidate will possess a strong commitment to providing high-quality patient care across various settings. As an LVN, you will play a crucial role in supporting patients' health needs and collaborating with healthcare professionals to deliver exceptional care.
Duties
Provide direct patient care under the supervision of registered nurses and physicians.
Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
Monitor patients' vital signs and report any changes or concerns to the healthcare team.
Document and report any changes in a patient's condition, medications, or treatment needs.
Assist in wound care, dressing changes, and managing other health needs.
Communicate with patients, families, and the healthcare team about care plans and any concerns.
Maintain a clean and safe environment for patients, adhering to infection control protocols.
Supervise certified nursing assistants (CNAs) and other support staff.
Experience
Candidates should have experience or training in the following areas:
Minimum 1 year experience as LVN, preferred SNF experience
Experience with Point Click Care a must
Geriatrics
Memory Care- Dementia and Alzheimer's Disease
Wound Care
CPR
A valid LVN license is required, along with a commitment to continuous professional development in nursing practices. Applicants should demonstrate strong communication skills, empathy, and the ability to work effectively within a team-oriented environment.

Pay: $31.00 - $35.00 per hour
