Job Summary
Meadowbrook Post Acute is seeking compassionate, dependable, and motivated Certified Nursing Assistants (CNAs) to join our nursing team. The CNA provides direct resident care under the supervision of licensed nursing staff and plays an essential role in promoting resident comfort, dignity, safety, independence, and quality of life.

Responsibilities
Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
Provide routine resident care, including obtaining vital signs, weights, and intake/output measurements as assigned.
Respond promptly and appropriately to call lights and resident requests.
Assist residents with transfers, repositioning, ambulation, and mobility using proper body mechanics and facility-approved techniques.
Observe residents for changes in physical, mental, emotional, or behavioral condition and promptly report concerns to the licensed nurse.
Provide compassionate, respectful, and person-centered care while protecting resident privacy and dignity.
Maintain a clean, safe, comfortable, and organized resident environment.
Assist with meals and encourage adequate nutrition and hydration as directed.
Follow established infection prevention, safety, emergency, and resident rights procedures.
Complete required documentation accurately and timely in accordance with facility policies.
Work cooperatively with nurses, rehabilitation staff, dietary, activities, social services, and other members of the interdisciplinary team.
Participate in facility training and in-service education as required.
Perform other duties within the CNA scope of practice as assigned.
Qualifications
Current Certified Nursing Assistant (CNA) certification in the State of California.
Current CPR/BLS certification preferred.
Previous experience in a skilled nursing facility, long-term care, post-acute care, or other healthcare setting preferred.
Strong communication and interpersonal skills.
Dependable attendance and punctuality.
Ability to work effectively as part of a multidisciplinary team.
Compassionate and resident-centered approach to care.
Ability to prioritize tasks and respond appropriately to changing resident needs.
Ability to safely perform physical job duties, including standing, walking, bending, repositioning, and assisting with transfers.
Commitment to maintaining resident confidentiality, dignity, and rights.
