About This Position

Job Summary

We are seeking a dedicated and detail-oriented Resident Assessment - Minimum Data Set (MDS) Coordinator to join our healthcare team. The successful candidate will be responsible for coordinating and completing resident assessments in compliance with federal and state regulations, ensuring accurate documentation for quality care planning. This role requires a Registered Nurse with a strong understanding of long-term care assessments and regulations. The MDS Coordinator plays a vital role in optimizing resident care through precise data collection and assessment processes.

Responsibilities

  • Conduct comprehensive resident assessments in accordance with federal, state, and facility guidelines, utilizing the Minimum Data Set (MDS) process.
  • Collaborate with interdisciplinary team members to gather relevant clinical information and ensure accurate documentation of resident status.
  • Ensure timely completion and submission of MDS assessments to support appropriate care planning and reimbursement processes.
  • Maintain up-to-date knowledge of regulations related to resident assessments and coordinate compliance efforts across the facility.
  • Provide education and support to staff regarding assessment procedures, documentation standards, and regulatory requirements.
  • Review resident records for accuracy, completeness, and consistency, updating assessments as needed during quarterly reviews or upon significant change in condition.
  • Assist with quality assurance activities related to resident data collection and documentation accuracy.
  • Act as a liaison between clinical staff, administration, and regulatory agencies concerning assessment-related matters.

    Qualifications

  • Registered Nurse (RN) licensure required.
  • Prior experience with MDS assessments or working within long-term care settings is preferred.
  • Strong knowledge of federal and state regulations governing resident assessments and documentation standards.
  • Excellent organizational skills with attention to detail to ensure accuracy in documentation.
  • Ability to work collaboratively within an interdisciplinary team environment.
  • Good communication skills to effectively educate staff and coordinate assessment activities.
  • Proficiency in electronic health records (EHR) systems and assessment software is desirable.
